Steve,

This is pretty much what you propse (which I like) but uses 3 tags instead of 4. [Just a variant, really.]:

1 - Define as a container of s (or s, if you prefer):

    ... 
    ... 
    ... 
      ...
    ... 

2 - define (or ) to contain a and an :

    ... 
   
      ...
   

where maybe empty (i.e )

3 - define to contain text.

headline text goes here...

For example:

   
      head 1
      
         
            head 1.1
            
            
         
            ...
         
            head 1.m
            
         
      
   
      ...
   
      head n
      
         
            head n.1
         
            ...
         
            head n.m
         
      
   

and so on.
