nirvana not serving images

apologies if this has been covered - http://search.userland.com/default?m=25&s=1&q=images+guest+database came up with nothing really helpful. i've gone through the checklist, and things look fine there.

the server seems to find the image object just fine, and the html looks good, with a relative ref to "images/imagename.ext" (even though #noImageExtensions is set to true). however, the page shows up with a broken image icon.

everything's fresh and new here, running 5.1.6 on a mac, with all the bits updated.
