Actually, The Well has had a lot of success just by not letting people post anonymously. People think a lot harder about their words when their name is associated with them. It also helps that you're paying $10/month to use the system. Most jerks aren't willing to pay that kind of money just to be jerks.

Slashdot's self-moderation is a poor second, and requires a lot of oversight to avoid abuse.
